We have a fix for this bug ready and it will be included with the next game update. Until that time, please try using the workaround posted above by Gaile. If this workaround is not working for you, the fix in the next game update will hopefully solve the issue.
There was an update released today (11-Aug-2015). I don’t see anything about title or Hall of Monuments fixes in the release notes.
Was it just missed from the release notes, or did this fix not make it into today’s update?
Note: I used the “search” function but didn’t turn up any previous reports of this issue, which I find highly suspect…
I have twice accidentally kicked a player from the guild I’m in, solely because of a slight miss with the mouse when right clicking on their portrait when partied up with them. There’s no confirmation dialog at all — if you miss the menu item you were trying for and accidentally hit “Kick from Guild”, they’re just immediately gone.
The first time this happened, I was trying to ’Kick from Party" after they had forgotten to leave party when logging out. The second time, I was trying to join them in non-overflow. Both times, a tiny miss with the mouse lead to their instant dismissal from the guild.
To add insult to injury, there appears to be some kind of a cool-down when accepting a re-invite to a guild. In both situations, I immediately sent a new guild invite to the player and an apology, but they were unable to accept the guild invite for more than 24 hours.
Please fix this! An action like this should have a confirmation dialog box, to ensure that this is really what you intend to do.

This is definitely a nasty UI issue. Please make it a priority to fix this!
I’m having the same problem. I played for about an hour on Friday night until I realized I wasn’t getting any credit. I reported the bug in game and went on to something else.
I gave it a try again today. Got in on a partial game and then played one full round, where I had a great game (winner, most grabs, most carries, most eliminations, 213 points) and still didn’t get any of the living story crab-toss achievements to update and I didn’t get the daily achievement point for winning the game.
If I knew for certain that the totals were being recorded but just not being reflected in the achievements tab (yet), I would continue playing, but without knowing that I can’t really justify continued play in the mini-game. I’m only doing it for the achievement points, and if I’m not getting them, I should be doing something else.
Ask any database administrator, duplicate records are not a good thing. With what you are suggesting is have two sets of the same information, now how do you handle things like gaining a level do you update the home server, guesting server or both servers.
It’s called master-master replication and lots of databases and technologies that use databases use it. It’s certainly more complicated that single-master or non-replicated, but it’s not alien technology. It’s widely used, by companies a lot less tech-saavy than Arenanet.
Understand that I’m not advocating copying everything between both datacenters, just refuting your statement that it’s harmful or dangerous or some giant technical hurdle. It’s not.
