Go with Purchase Issue > Upgrading your account (or Purchase Issue > Refund > Account Upgrade).
Then title it so that the situation is obvious, ie. that you bought the core game recently at 40$ and now HoT offers it essentially for free.
Without more specific official instructions you can always reference the dev statement by GM Proheals on reddit if you hit a brick wall with CS.

Once you have a resolution on this, you might want to post here how it turned out so others can use it as reference. In case ArenaNet decides not to make public statement on the issue.
