There was a bug at the time the Mad King Says was released, it did not reward the mask properly to everyone if I remember correctly. This was fixed in a day or two, but you must have done it early on and didn’t repeat it after the fix.

what about dungeons? There are some PoI’s in dungeons if memory serves. Do they count towards world completion?

I have the same issue. This also affects some (most?) random drops, the most noticable being the hat slot, as most hats display this weird looking cowl for light armour. I can see dungeon armour well enough on other people, just not on me.
