Had this happen to me tonight, killed Tequatl, chest didn’t spawn. Prior to killing the dragon, I took a few hours break from playing, and before that I had simply leveled an alt from 44-46, doing events.
Windowed mode tanks performance, unfortunately.
Dual screen issues in fullscreen are a common problem with mac ported games like this, there’s not much hope of your second monitor ever being usable while playing fullscreen.
