Thanks for the reply Michael. The only issue I have with getting this to work, is that the bank slots were not actually purchased in the gem store. They were purchased from the bank window, thus do not show up on the recent gem purchases area.
Here is the response I got some AN on this issue:

I’m afraid that we are not currently able to refund, remove, exchange, or compensate you for purchases made using this system currently regardless of the reason. I understand this may be frustrating, but we simply do not have the capabilities in place to complete such a transaction at this time. Again we apologize for any frustration this causes, and we hope that you can still continue having fun playing the game.

And here is my response to this answer:
“As of right now, you cannot handle this issue. However, am I allowed to keep my ticket open so when the day does come that you can fix this situation, you could fix the situation then?
I would like this ticket to remain open until your team does have a way to fix this issue, and promptly fix the issue then please.
I spent real world money on the gem cards that I added to my account. Each card was for 1200 credits and cost $15. This bug is indeed an issue that has cost my $7.50 of real world money. This isn’t a convenience issue. This is an issue of money being taken away from me without the power to get reimbursed, through no fault of my own.
I am willing to wait.
Is this an option?”

I never receive a receipt for buying items from the Gem Store with gems. Does anyone?
I receive a receipt if I purchased gems via the in-game store with my credit card, but for nothing else.
And, I do not want a monetary refund. I simply want them to take away the extra bank slot, which has no items in it, and to refund me the 600 gems.
And, like I said, I did submit an in-game bug report once it happened, so AN should have all of the relevant information that they need to find out if this is a known issue or something new.
Has anyone else experienced this issue before?
Also, I know that I did not buy two by mistake because every time you click to purchase a new tab, it gives you a confirmation window. During the transaction in question, I only received one confirmation window.
This is clearly a bug.
And, I used my real world money to purchase these gems. This is a direct loss of money for me.
I attempted to purchase 1 bank slot last night for 600 gems. Once I click the button to confirm the purchase, it charged me 1200 gems and gave me 2 bank slots.
Is there anyway to get the extra slot taken away and the gems returned? I submitted a bug report in game already, but no response on it.

Thank you all for the updates. It seems this was fixed without any word from the devs. Please close this thread. I will re-open another one if this is encountered again.
lol. completed CoF story last day with little to none problem at all. killed this boss on the first try without any casualties.
Then please tell us the strategy that you used to get around the issue of him resetting so easily now?
Before the patch, this boss was a non-issue for my group.
Maybe they stealth fixed it with the latest patch put in yesterday. I haven’t run it since.
Can we get some info from a dev if this was an issue that has been resolved then?
New patch notes came out and no mention of this.
The dungeon is unbeatable in it’s current state. You are locking people out of this dungeon, but also all of the exploreable wings because the story must be completed first.
Can we at least get an acknowledgement that this is an issue that is known now?
Watching some youtube videos of this fight before the recent patch confirm for me that he is resetting much easier now.
Can we get some kind of response on this please?
Until this is resolved, the fight is broken, thus the story mode is not complete-able.
One more note for this fight.
The issue isn’t that we are taking the boss too far away from the room. When he does his GS spin, he goes past us. Thus, if you are standing in the general direction of where he could reset, he will spin through you, thus putting himself into an area that he will reset in.
Any chance we can get a response from a dev on this? Is this fight working as intended? Why was it changed? Is it simply a bug?
Rozgar the Forge is unbeatable in COF story since the latest patch.
Before this patch, my guild has beaten him on a regular basis running guildies through the story mode so we can get ready to do exploreable.
Since the patch, he resets far to easily. It makes you fight him in the room he spawn in. However, this room is far too small. He starts every fight in his greatsword mode with 2 GS pets. With all 3 of these doing their whirls in GS, they are able to cover approx 85% of the room. There is no way to dodge all of the attacks.
Going outside of the room will cause him to reset very quickly because he will aim an attack down the ramp, and reset because he is too far away from the room.
I looked through the patch notes, and I saw nothing about CoF except for fixing map exploits. Is this the way that AN intends for us to fight him? If not, is this a known issue?
