SPOILER AHEAD
After finishing it on 2 characters I tried the last part of the HoT personal story three times with another one:
1) Disconnect when Mordremoth was at 50%.
2) & 3) So, 2 tries in a row, when I defeated the Pale Tree, she vanished and clicking the portal did nothing. Seems, that other players got stuck this way in the instance too.
