Same here, I am getting the following error message:

Having the same problem. I have openin strike and i was in a cave with another Ranger having Opening Strike too. We attacked and stacked 10 Vulnerability. Than i went out of combat got the Opening Strike icon on the botton and after my next attack on another mob it didnt trigger. Randomly appearing to trigger i didn’t notice any scheme or repetition.
