I have experianced both of these issues, however, I experiance it with the ori picks.
From what I can tell is, you get 1 hit on the vien when you hit it just around the time it should respawn, try waiting 10-20 minutes and then check it, the vien should be there if not wait a little bit longer thats what I have always done.
for the rich mithril, last time I had that happen was when I went to farm the vien, then my net died, I did not fully farm it, and got back to it the other day, it should have reset, but it did not it seems that you will not get a reset unless you fully farm the vien.

This is indeed an interesting topic, I have been following the info being posted here and found out some stuff based on trial and error in game. I also feel that this is actually not a bug.
From the way I understand this. Transmutation is a good thing, but at the same time a bad thing. When you transmute an item your given Three choice once you put the Two items in that you want to transmute.
Choice number 1:
This choice will allow you to transmute the skin of the weapon, when going from a normal sword skin to say the krytan sword skin. Now in terms of Legendary transmutation you would want to keep the skin of the Legendary weapon your transmuting.
Choice number 2:
This is the big choice, the choice of the Stats, but do not let this fool you if you take a closer look at the window of what you are selecting, there is another piece of information in there.
I am going to use the Twilight as the Legendary in this explanation, and Dusk as the Exotic
Item number 1 Twilight
179 Power
128 Toughness
128 Vitality
Item number 2 Dusk
179 Power
128 Precision
9% Critical Damage
Choice number 3:
Sigil
For a DPS build I would want the stats of the Dusk over the stats of Twilight, so I would Transmute the items based on the Three principals of transmutation.
1: Skin = Twilight
2: Stats = Dusk stats
3: Sigil = your sigil
End result is the weapon having the skin of Twilight and stats of Dusk but having a rarity of Exotic versus a rarity of Legendary.
So here is where the confusion is, why is my weapon now an Exotic weapon and not a Levendary weapon???
There is an explaination for this, and this explation will also explain how I am seeing that this is unfortunately Not A Bug.

To explain what is going on here, and where the confusion is, we need to take a closer look at Transmutation Choice Number 2.

Now, the key to this is to not be focused on the attack, and the stats alone.

Ok so what I am trying to do in this explaination is to show you that upon transmutation you have more options in Choice Number 2 than most realize, one of the options is KEY.
The items skin my be Legendary or Exotic, but that is not what determines the rairty of the item, the rairty of the item is based off of Choice Number 2, where it states in the stats window not only the stats, but the rarity of the item.
So transmuting a Twilight with a Dusk to get the Dusk stats, results in transmuting a Legendary stated item, with a Exotic stated item, thus resulting in having the sking of Twilight, the stats, and the rarity lvl of Dusk. Effectively making an Exotic Twilight.
Basically it is not the skin that makes the Legendary but the stats, and that can be misleading because when you put it in the skin choice has the skin and name the color of its rarity, but its not the skin choice that determines the rarity its the stats choice.
