You access bankslot upgrades at your bank. There is a lock icon in the lower right corner that you can doubleclick to purchase slots.
Some items are non salvageable, in particular, those items that are obtained from karma vendors. (although there are a few others I’ve seen.)
Do bag slot gems get refunded upon character deletion?

No they don’t. Try to be sure that if you buy slots, it is on a character you will be playing long enough to get good use out of it.
I have seen this issue with blue+ colored gems, and other items that are NOT soulbound.
If the items were purchased with karma you cannot salvage them.
