This just happened to me too. Lost the key, and no loot.
For “A Study in Gold” I didn’t get credit for #9 and #10. Haven’t tried any of them above that.
In the achievement “A Study in Gold” there’s an error in #7 “A Great View of a Mighty Tree”. When I approached this one, I got a checkmark for #8 “Southgate: Tucked Away Southwest” instead of a checkmark for the #7 slot.
I just got this while doing Temple of the Forgotten God, so it’s definitely not an issue of the number of players(I was even solo – no party members at all).
I couldn’t use skills 1-5, I could use 6-0, I couldn’t evade/use adrenaline.
It happened during/after a battle where I was underwater killing some risen. I went to go pick up an Illusory Essence that I had dropped for the battle, and was unable to pick it up again.
I ended up having to relog, because nothing I did enabled my skills(unequipping items didn’t work, I could pick up other objects in the mission, and I couldn’t die due to killing everything hostile)

I have the same issue.
Using a 260 GTX, I’ve tried the most recent(non-beta) drivers, as well as all beta driver versions since, to no avail.
Q9550 OCed to 4 GHz
4 GB DDR2 RAM
128GB SSD with 40 GB remaining, plus 2 TB traditional HDD storage space
It’s a bit of a complicated build, but I’ve tried running off the SSD as well as one of the HDD. Since other people have the same issue, I’m assuming it’s not an issue with corrupted SSD files(which has happened in the past, very infrequently).
My graphics card is well ventilated, and never runs into its ‘hot’ zone. I have graphics on the lowest settings, forcing windowed mode and 60 fps.
I’m coming to the same conclusion as Xenos. It’s a memory issue, and for whatever reason GW2 doesn’t like the 32 bit system with only 4 GB of RAM, even though the min spec suggests something like 2GB is ok. I technically have 8 GB of RAM in my computer, and I used to run Vista 64-bit, but now I’m on 32 bit Windows 7 so I don’t see all of it anymore.
GW2 is the only thing that gives my computer trouble(as far as crashing, I know I need a new graphics card to get the best of DX11, but I also need DDR3 memory, which means a new mobo, and I don’t need a new processor, but I’ll be forced to get one…essentially any upgrade for me is a whole new computer). The Q9550 I have is never put through its paces. Its been stable at 4 GHz since it came out, which was several years ago.
Yeah, I think its memory. Maybe a 64 bit system with all 8 GB would help. It could be the SSD, but I’m placing that around ~10% probability.
**Edit: I just noticed nVidia has a new official driver out, 306.23. Some people have mentioned success with these new drivers. I’ll report back if they’re good.
